Application Forms

The Application Guidelines will give you information on how you can apply for one or more of the nine categories of awards and which application form you will need to complete.

If you would like help in completing the application Equality North East will be delighted to assist.

Application Form A

should by completed for the following categories:

• Employers of Between 51 and 250 Staff
• Employers of Between 11 and 50 Staff
• Employers of Up to 10 Staff
• Learning Providers Up to 50 Staff
• Learning Providers & Colleges of Over 50 Staff

Application Form B

should by completed for the category of

• �Larger Organisations Who Have Made an Outstanding Contribution in Promoting Equality & Diversity in the Community�.

Application Form C

• 'The TUC ~ unionlearn Award'

Application Form D

should by completed for the category of

• �Individuals Who Have Made a Difference�

Application Form E

should by completed for the category of

• �Innovative Working Practices�

Application forms and monitoring forms must be completed and returned by close of business, Monday 30th June 2008.
